Do you know a TV or film professional or a behavioral health consumer/peer leader who has been a key player in educating the public about behavioral health (mental health and/or addiction issues)? If so, please help SAMHSA recognize him or her by nominating his or her work for a 2011 Voice Award .

Nominating Consumer/Peer Leaders
If you know of a consumer/peer leader who has led efforts to promote the social inclusion of individuals with trauma histories and behavioral health problems, personally demonstrated that recovery is real and possible, and made a positive impact on his/her community, workplace, or school, please nominate him/her for a 2011 Voice Award.

For the purposes of the Voice Awards, the term "consumer/peer leader" is defined broadly as an individual who has received or is receiving services for a mental health and/or addiction issue.

To make a nomination, please submit the following:

  • Consumer/Peer leader's name and contact information
  • Your name and contact information
  • A description (500-word maximum) of the nominated consumer/peer leader's contributions as they relate to the three criteria listed above.

Please mail your nomination to the address below or e-mail the requested information to voiceawards@vancomm.com . Nominations are open to anyone and are free. There is no limit to the number of nominations an individual can submit. Self-nominations are welcome.

Nominating Writers and Producers
If you know of a TV or film production that was released after April 15, 2010 and offers a dignified, respectful, and accurate portrayal of individuals with trauma histories and behavioral health problems, please nominate it for a 2011 Voice Award.

To make a nomination, please tell us where and when you saw the production you wish to nominate. If possible, please also indicate the following information:

  • Title of production
  • Production medium
  • TV or film
  • Format of production
  • Series, episode, feature, or documentary
  • Type of production
  • Drama, comedy, reality, other (please specify)
  • Date and time of initial public release
  • For TV: Date and time the program aired
  • For film: Movie studio
  • Name(s) of writer(s) or producer(s)
  • Episode name or number (if applicable)
  • Reason for nomination (you may attach one additional page if you need more room)
  • Your contact information (OPTIONAL--you may nominate anonymously)
  • Name, address, phone, and/or e-mail

When nominating, please be as specific as possible, and if available, forward a DVD of the production you are nominating. At a minimum, include the production's title, date, and outlet (TV network or channel, cable network or channel, movie studio, etc.) of initial public release.
